 At PT Center for Women, we are always accepting new patients! We offer convenient ways to schedule an appointment or make changes to an existing appointment. Call/Text If you would rather make an appointment by phone, you can call or text us at 920.729.2982. APPOINTMENT INFORMATION Your first appointment For your first appointment, we ask that you bring a photo ID and your insurance card. We also ask that you arrive 10 minutes early to complete any necessary forms or to update your information. Insurance card It is important to bring your insurance card to every appointment to verify changes have not been made, as well as to make sure we continue to participate with your current insurance plan. Need to cancel/reschedule an appointment? Should you need to cancel or reschedule an appointment, we kindly request you do so at least 24 hours prior so we can schedule other patients who may be waiting for an appointment. Any patient who fails to show or cancels/reschedules an appointment and has not contacted our office with at least 24 hours notice will be charged a $50 fee. |
