As a courtesy to you, we accept most insurance plans and will submit your claims accordingly. However, we encourage you to verify your benefits with your insurance company prior to your appointment. This will ensure you'll receive the maximum benefits available to you. Accepted insurance carriers include, but are not limited to:
Please bring your insurance card to each and every visit! Your insurance plan may require that you pay a co-payment, a deductible, or a co-insurance (part of the bill). All co-payments are due at the time of service. If you are unable to make your co-payment, your appointment may be rescheduled. For any questions about your financial responsibilities or benefits, you should contact your Human Resources department or call the customer service number listed on the back of your medical insurance card. |
