Insurance and Billing

As a courtesy to you, we accept most insurance plans and will submit your claims accordingly. However, we encourage you to verify your benefits with your insurance company prior to your appointment - this will ensure you'll receive the maximum benefits available to you. Accepted insurance carriers include, but are not limited to:
  • Aetna
  • Anthem BlueCross BlueShield
  • HPS Meritain
  • Humana
  • Medicaid
  • Medicare
  • UnitedHealthcare (UHC)
  • United Medical Resources (UMR)
  • WEA Trust
  • WPS/Arise
Your insurance plan may require that you pay a copayment, a deductible, or a coinsurance (part of the bill). All copayments are due at the time of service. If you are unable to make your copayment, your appointment may be rescheduled. For any questions about your financial responsibilities or benefits, you should contact your Human Resources department or call the Customer Service number listed on the back of your medical insurance card.

Please bring your insurance card to each visit. A sample insurance card is below highlighting the important information you need to know for your office visit. Please keep in mind that all insurance cards look different so the information may be in a different location on your card.


Billing
You will receive a monthly statement listing charges for services received, pending insurance claims, and any payments made in the last 30 days. Your financial responsibility will be listed in the patient balance column and payment in full is required by the date due. Payments are accepted in cash, check, or credit card. You may also pay online here. If you are unable to pay the amount in full, please contact our billing department by calling 920.729.7105. Payments can be mailed to:

We do accept CareCredit® as a form of payment. CareCredit® is a healthcare credit card that can be used as a payment option for certain expenses not covered by insurance or to bridge situations when desired care exceeds insurance coverage. Payments using CareCredit® must be done in person and you will need your CareCredit® card, as well as two other forms of identification.
